Abstract : Background: For many years, the most advantageous labor position has been questioned. The lithotomy position is the position most commonly used in tertiary settings; however, recent recommendations advise sitting position. Labor position during the second stage of labor affects both maternal and fetal outcomes. Aim: to compare the effectiveness of lithotomy and sitting positions during the second stage of labor, from the perspective of maternal and fetal outcomes. Design: A quasi-experimental design was used following a purposeful sampling method. Sample size: 120 low risks primiparous divided equally into sitting and lithotomy positions. Setting: Labor and delivery unit at KAUH, Jeddah. Sampling: Data collected over a six-month period from January to June 2020. Tool: A structured questionnaire divided into five parts; Part I: socio-demographic data. Part II: maternal and fetal initial assessment. Part III: maternal outcomes checklist: includes perineum condition after birth, duration of the second stage, and mode of delivery. Part IV: fetal outcomes checklist. Part V: maternal satisfaction with labor position. Results: the results show a significant positive effect when using the sitting position; i.e. reduced episiotomy rate, shortening of the second stage of labor, facilitate the spontaneous vaginal delivery, newborn arterial cord pH, Apgar score at one and fifth minutes of life, newborn transfer to the intensive care unit, and maternal satisfaction at the p-value <0.05. Conclusion: Use of the sitting position during the second stage of labor has a positive effect on maternal and fetal outcomes when compared to the lithotomy position. Recommendation: Women should have the right to be educated about the benefits of adopting the sitting position during the second stage of labor.
